Click on the form you wish to open.

Please note that completing and submitting a form is the first step to your request.
Approval for an event depends on the availability of the church building and pastor.
If you do not hear from the church office within two business days of submitting your form and request, please call (215) 886-4612  ext. 3.

Calendar availability

We ask that you check the church calendar for date availability before submitting a form.
Click here to view the church’s web calendar. The church office makes every effort to update the web calendar on a daily basis. There is the possibility that a previously scheduled event may not be posted to the web calendar due to technical difficulties. Additional information may be sent to you upon receiving your request.

Submitting a form

1.  Please open and complete the form you wish to submit.

2. Save the completed form to your desk top, using your name and form name as the document name.

Example for naming the form and saving your document to be submitted:

John Smith_Baptism_date of baptism
Name of Organization or Your Name_Facilities Request_date of the event
John Smith Family_Member Info_today’s date
Spouse Name and Spouse Name_Wedding_date of wedding

3. Open the form on your desk top to see that the fields have been saved. Then close the form.

4. Email the completed form as a PDF attachment to the church office with your full contact information and a brief explanation of your request in the content of the email to:
